In-Home Assessment: $120

Telehealth Assessment: $85

Key Responsibilities
-
Conduct annual health assessments for high-risk patients
-
Collaborate with multidisciplinary healthcare teams
-
Screen patients for participation in care management programs
-
Advocate for patients while maintaining privacy and confidentiality
-
Provide patient education on health management and community resources
-
Facilitate patient empowerment and promote independent, informed care decisions
-
Identify opportunities for health promotion and illness prevention

Complete documentation within 48 hours of each visit

Communicate clinical escalations to the Team Manager promptly
Required Qualifications
-
Active, unencumbered RN License
-
Nurse Practitioner Certification from ANCC or AANP in Family, Adult, Gerontology, or Emergency Medicine

Board Certified through AANP or ANCC
-
Current CPR Certification
-
Compliance with standard healthcare vaccinations and preventive testing (Hepatitis B, Influenza, MMR, Tdap, Varicella, Tuberculosis)

Ability to communicate effectively with elderly and chronically ill patients
Preferred Skills & Experience

Home health experience

Knowledge of Medicare, Medicaid, and Health Plan benefit structures

Strong patient interaction and communication skills

Ability to multitask and provide excellent customer service
