  • Coordinate with community resources and VA programs to connect Veterans with additional support.

  • Participate in team meetings, care planning conferences, and patient triaging to ensure smooth and effective service delivery.

Qualifications

  • Be a U.S. Citizen.

  • Hold a Master’s degree in Social Work (MSW) from a CSWE-accredited program.

  • Possess a valid state license to practice social work at the independent level.
