Position Overview
As a Social Worker in the PC-MHI program, you will work at the intersection of primary care and mental health services. Your role will focus on short-term, solution-oriented care—providing counseling, assessments, and behavioral interventions to Veterans in need. Key Responsibilities
In this position, you will:

Conduct 20–30 minute functional assessments to identify challenges and determine the appropriate level of care.

Provide brief, evidence-based interventions, such as CBT for insomnia or chronic pain, grief counseling, and motivational interviewing.

Work closely with primary care providers and PACT team members to create integrated care plans.

Coordinate with community resources and VA programs to connect Veterans with additional support.

Participate in team meetings, care planning conferences, and patient triaging to ensure smooth and effective service delivery.
Qualifications
To qualify, you must:

Be a U.S. Citizen.

Hold a Master’s degree in Social Work (MSW) from a CSWE-accredited program.

Possess a valid state license to practice social work at the independent level.

Have experience or strong knowledge in medical terminology, community resource coordination, and mental health care.
