🧾 Position Summary
The RN Diabetes Educator provides comprehensive nursing care and education to patients with Type 1, Type 2, and Gestational Diabetes. This role focuses on empowering patients to self-manage their condition through personalized education, hands-on device training, goal setting, and emotional support.
You will work collaboratively with physicians, clinicians, and educators to deliver a multidisciplinary approach to diabetes management.
✅ Key Responsibilities & Accountabilities
📚 Patient Education

Educate new, returning, and follow-up patients on diabetes management

Support insulin pump and continuous glucose monitor (CGM) users

Teach blood sugar monitoring, nutrition planning, and coping strategies

Provide education for gestational diabetes patients

Conduct phone follow-ups to address patient concerns
👩⚕️ Clinical Collaboration

Work closely with endocrinologists and care teams

Assist in teaching diabetes-related classes

Support outreach programs for Urgent Care sites

Help develop diabetes programs at additional care sites
📞 Communication & Outreach

Follow up with patients who miss endocrinology appointments

Coordinate with community diabetes outreach programs

Meet with pharmaceutical representatives regarding diabetes products
📈 Quality Improvement

Assist in developing diabetes education programs across ambulatory sites

Create and update patient education materials

Assess and instruct patients on new diabetes equipment and technology
🎓 Education, Licensure & Certification Requirements
✔ Required

Graduate of an accredited school of nursing

Valid California RN License
-
BLS (Basic Life Support) Certification

Certified Diabetes Educator (CDE)

Experience in OB/GYN or gestational diabetes care

Minimum 5 years of recent relevant nursing experience
